Overall Responsibilities:
Under the supervision of the Associate Executive Director, the Community Resource Navigator will represent Healthy Start Coalition in community events and meetings, as well as oversee CONNECT/CIR related tasks and responsibilities. Additionally, will represent Healthy Start Coalition in Educational visits to community providers; and agencies to assist in advocating as to the importance of early intervention for Healthy Start screening process.
Principal Duties:
· Attend identified community meetings, and provide updates regarding Healthy Start events;
· Ensure education of new programs relevant to Healthy Start Program is provided to team, either during weekly meetings, and or other opportunities;
· Provide educational visits to provider offices throughout Sarasota County, offer education and screening data to better inform practices, must complete 6 visits at minimum per month;
· Provide educational materials to provider offices during visits that support and promote healthy outcomes related to pregnancy, infant, and child health and wellbeing;
· Be main point of contact between Healthy Start Sarasota and community Home Visitation Programs;
· Provide Supervisor with proposed schedule, for outreach and educational activities;
· Ensure that Supervisor is kept informed on networking and building connections within community;
· Provide educational opportunities on programs of Healthy Start, to be provided by self, or the program specific staff person depending on the audience and the level of education requested;
· Create calendar of national awareness topics related to prenatal care, maternal health, infant and child health, fatherhood, minority health, etc. Provide educational, social media postings that can be used to educate and inform;
· Adhere to any outcomes related to performance and activities related to job duties;
· Facilitate identified meetings within timeframes as outlined in activities listing;
· Daily assignment of CIR cases; and triage assistance as necessary;
· Maintain positive working relationships with partners and stakeholders involved with Healthy Start System of Care;
· Assist with identification of service delivery gaps for population served;
· Carry a case load, to be technical assistance;
· Assist with training new CIR/Connect staff as needed;
· Other duties as identified or assigned.
Skills Knowledge:
· BA in Human Service related area of study;
· Ability to multi-task and handle multiple duties at same time;
· Communication skills; both verbal, and written;
· Ability to work as a team;
· Motivational Interviewing Skills a plus;
